To send a form using webmail such as yahoo, gmail or hotmail, you must have a fillable and savable pdf form.
 
  1. Make certain you have Free Adobe Reader version 8 or 9.  Version 7 might work.  If you're on a Mac, change the default viewer for pdf from "preview" to Free Adobe Reader.
  2. Fill the form and save the filled form to a place you can find it.  Desktop is convenient.
  3. Open your email and start a message.
